How to Buy an Athena PMT

The cost of the Athena PMT is $724. Many insurance companies cover most of this. If you do not have insurance we have special programs to help you obtain the Athena.

The Athena PMT is available by prescription only.

Since the Athena PMT is a very new product many doctors are not yet familiar with it. The form package includes an informational brochure to print and give to your doctor if you wish.






	Locate a prescriber


	Take these materials to your doctor and request that he/she prescribe one for you. If your doctor has questions many answers can be found in the Medical section of our web site or we can mail information to him/her.

	All orders must be accompanied by photocopies of any insurance cards and a completed Letter of Medical Necessity. If you have insurance, Athena will coordinate the billing with your insurance and will inform you of the costs before processing your order. All costs of the product not paid for by your insurance company will become your responsibility. Please review the Purchase Agreement on the Order Form for terms and conditions of sale.
